چکیده

COMMUNICATIONS TO THE EDITOR 681 resenting supraventricular tachycardia with aberrant ventricular conduction. Rapid administration of digitalis was started, and one hour later the rate dropped abruptly to 50 beats per minute. Intravenous therapy with isoproterenol, dopamine, and sodium bicarbonate was administered, and the rate varied between 70 and 160 beats per minute. Eight hours after admission, a consultant in cardiology reviewed the patient’s electrocardiograms and course and believed that the findings were best explained by adrenal insufficiency with hyperkalemia, despite normal levels of sodium and potassium at admission, tachyarrhythmia rather than bradyarrhythmia, and clear absence of significant dehydration. Determinations of serum electrolyte levels were reordered and revealed a sodium level of 123 mEq/L and a potassium level of 9.6 mEq/L. Corticosteroids were administered intravenously. A variety of disturbances in rhythm occurred, including atrial flutter with rapid and slow ventricular responses. A transvenous atrial pacemaker was inserted, which produced ventricular capture at a rate of 160 beats per minute. An hour later, asystole that was unresponsive to the pacemaker occurred, and the infant was pronounced dead. On the following morning the sample of serum obtained at admission, from which the normal values for sodium and potassium levels had previously been obtained, was located, and the determinations were repeated. On this occasion the serum sodium level was found to be 122 mEq/L, and the serum potassium level was 10.1 mEq/L. At necropsy the adrenal glands of this patient were found to be approximately the same size as the kidneys (a combined adrenal weight of 18.6 gm).

Toluene inducing acute respiratory failure in a spray paint sniffer

BACKGROUND Toluene, formerly known as toluol, is an aromatic hydrocarbon that is widely used as an industrial feedstock and as a solvent. Like other solvents, toluene is sometimes also used as an inhalant drug for its intoxicating properties. It has potential to cause multiple effects in the body including death. CASE REPORT I report a case of a 27-year-old male, chronic spray paint sniffer, ...

Encephalopathy and Neuropathy due to Glue, Paint Thinner, and Gasoline Sniffing in Trinidad and Tobago-MRI Findings

A 29-year-old male petrol station pump attendant was admitted with ataxia and clinical evidence of a sensorimotor polyneuropathy which developed over the preceding 3 months. He had cognitive dysfunction, hearing loss, and cerebellar clinical abnormalities that came on slowly over the three years. He had a fifteen-year history of sniffing mostly glue, occasionally paint thinners, and, in the rec...

Postural tremor induced by paint sniffing.

Volatile substance abuse is the intentional inhalation of volatile solvents, aerosols, gases or nitrates for the purpose of intoxication. This practice is more common among young people, due, in part, to the low cost and ready availability of these inhalants. In this report, we present the case of a 22-year-old male with a seven-year history of chronic paint sniffing. The patient presented with...

Noninvasive ventilatory support does not facilitate recovery from acute respiratory failure in chronic obstructive pulmonary disease.

This investigation evaluates, in a prospective, randomized and controlled manner, whether noninvasive ventilatory support (NIVS) with bilevel positive airway pressure (BiPAP) facilitates recovery from acute respiratory failure (ARF) in patients with chronic obstructive pulmonary disease (COPD). Twenty four patients (mean age (+/-SEM) 68 +/- 2 yrs) with COPD (forced expiratory volume in one seco...
